Youth Personal Supports Services

We believe every young person deserves the opportunity to live a meaningful, independent, and fulfilling life and to be supported in the least restrictive environment possible.

Our Youth Personal Supports Services offer one-on-one, individualized support for youth ages 13 and older with intellectual and developmental disabilities (IDDs) as well as those with co-occurring behavioral health needs.

Guided by each youth's Person-Centered Plan, Personal Supports assist youth and their families in developing foundational life skills, promoting resilience, increasing self-determination, and supporting successful transitions into adulthood.

Support happens where life happens: at home, in school, in the community, or even virtually using HIPAA-compliant real-time audio-visual technology.

Flexible Support

For Youth and Their Families

In-Person Supports

Our Youth Personal Supports provide one-on-one, hands-on support in the home and throughout the community, helping individuals build independence, develop new skills, and achieve their personal goals.

Virtual Supports

When appropriate, we also offer virtual supports that provide coaching, guidance, skill development, and ongoing connection through secure technology to increase independence while giving individuals and families greater flexibility.

Continuity of Support

When approved through the DDA waiver and identified in the Person-Centered Plan, our team can provide individualized supports during an acute hospital stay to help maintain routines, communication, and behavioral supports, and to ensure a smooth transition back to the community.
